 CASE OF THE FORTNIGHT #8

Presentation: 34- year - old female presented with postprandial bloating and abdominal distension, loss of weight and appetite. 


Intra-operative findings: Large splenic hemangioma occupying the upper 2/3rd of the spleen.


MRI: Enlarged spleen with well-defined, large, T2 hyperintense, lobulated, heterogenous mass, 10.5 x 10 x 8.5 cm. 


GROSS: A well circumscribed lobulated tumour, 10.1x9x8cm with pale tan firm to spongy cut surface and central pale white fibrotic areas with focal yellow areas.

24yr old female case of PCOD and infertility. Presented with vomiting, loss of appetite. H/o of herbal medicine for 2months S. Creatinine 11.3 mg/dl. Anemia: (HB 7.6gr/dl) Urine microscopy: Trace Proteinuria; WBC 5-6/HPF RPRF ?AIN Renal Biopsy performed.

 


Immunofluorescence negative for all antibodies ? Diagnosis

PATHOLOGISTS AS THE SHERLOCK HOLMES IN MEDICINE

  We walk through the Pathology lab as we are at 221B Bakers street the home of Sherlock Holmes for  want of history to come to a diagnosis. Many a time we do miss Mr. Watson in our work space to stitch  the missing link. As specimens enter and lie on our table with a smile,all we think of is a beautiful  description as we slice through as Holmes scored the leather in parallel cuts be it big or small. Not So Blue.....

 NOT SO BLUE  I know you not I know not your face. But I know your thoughts, With anxiety, ablaze.  I haven't met your eyes Yet I see the crab creep Through the mist and shadows Of your panic stricken sleep!!  It sweeps across you and me As an unexpected typhoon.
